As such I came back to it today and there is great info in the comments here. Maybe you need to do an e-book on writing e-books, pulling some of these ideas together :-) Anyway, J.D.'s idea to start with a deck instead of a word document was like a bright lightbulb to me. I went and downloaded his just to see how it worked. Thanks.
